Baptism Inquiry Form
The Bible says that we must be born of the water and the Spirit (John 3:5). It also says we must be baptized in the Name of Jesus Christ (Acts 2:38). Whether you've been seeking Jesus for a long time and never got baptized, not sure if you were baptized correctly, or are a new Christian who wants an experience with Jesus, we would be honored to baptize you in water and also pray for you to receive His Holy Spirit!
